I'm glad the mystery has been solved, but thought I would mention one other problem I'm aware of:

The temple recommends are serialized. There was a situation in our stake where the church sent the wrong book to a bishop. When the wrong stake was seen activating temple recommend serial numbers assigned to a different unit, they were deactivated (or maybe they never could be activated but it wasn't obvious they weren't working...I'm not exactly sure on the exact events).
