Bishops Food order web form fails

On Chrome, Edge, and Safari, every single time I hit 'Submit' or 'Save' on an online Bishops Food Order form, it fails. The following error appears each time.
"ISV code reduced the open transaction count. Custom plug-ins should not catch exceptions from OrganizationService calls and continue processing."

I've wasted more than 5 hours testing and trying different browsers and selecting different options. Would appreciate any help/fix

I was having the same issue. I spent about an hour trying different browsers and researching online with no success. Then I spent another hour testing different inputs on the form, i.e., different food quantities, selecting different members, non-unit members, different and no address, different phone numbers to notify the recipient, and it turned out to be the recipient's phone number. For whatever reason it didn't like the phone number, and it wasn't happy until I prefixed it with the "+1" US country code. So, +10005551234, 10005551234, 1(000) 555-1234, 1-000-555-1234, etc., worked.

Fixing the phone number solved my problem too! Thank you! (Must have +1 then a space before the phone number begins in the US). Basically, just don't change it from the way the system auto-populates the phone number.
