Attention to the Clerk, of Glen Innes Ward, Panmure Stake.
I am requesting my Patriarchal Blessing as I have lost it, about 15 years ago. I didn’t receive it in Panmure Stake. I had to go to the area East Tamaki, in Ōtara. I cannot evan remember the Patriarch who gave me my blessing. I cannot remember the date either. Just that the chapel I went too, was in Ōtara. I hope this helps you, to find it.

Your patriarchal blessing can probably be found via the Church's online request system: https://pb.churchofjesuschrist.org/pbrequest A form will ask for your personal information, perhaps your parents names, and any information you have on where and when the blessing was given. The page says it can take a few weeks for researchers to find your blessing. Once it is found, you will have online access to your blessing. You can also request blessings given to your deceased direct-line ancestors.
If you wish to contact the ward clerk directly, you can probably use the Ward Directory and Map app or web-app: https://directory.churchofjesuschrist.org/ For that, you'll need your Membership Record Number attached to your account.
If you wish to contact your bishop but don't know his name or contact information, the Meetinghouse Locator, https://maps.churchofjesuschrist.org/?lang=eng should allow you to send a message to him.
