I assume you're talking about a true 50/50 split, where both parents are still considered equal guardians of the child. I would suppose the Church is required to follow applicable local laws. So whatever is required for a normal school field trip, summer camp program, etc, would likely be the same as what the Church would do for permission slips.

Perhaps others in the Forum have personal knowledge about what to do in this situation.
